Dog Adoption Application

THANK YOU for your interest in adopting an older animal. Please complete this application, and a SPAAR representative will contact you if we feel your home is the right home for one of our adoptable animals.

Please note that SPAAR evaluates each application individually and does not have a set idea of what the ideal adopter looks like. We understand that each adopter is different just as the needs of each animal are different.

    Household Information

    Owners: If you do not live in Allegheny County, PA, you will be required to provide proof that you own your home (e.g., mortgage documentation).
    Renters: By completing this application, you authorize a SPAAR representative to contact your landlord and verify your ability to adopt.
    If you do not have a fenced-in yard, it does not in any way disqualify you from adopting. We appreciate your honesty.

    Reference Information

    Provide the following information for your current veterinarian (if applicable), employer, and someone not related to you that can attest to your character. If you do not have current pets and thus do not have a veterinarian, please list a second personal reference.
    SPAAR will not contact your employer without prior approval from you to do so; if we do contact your employer it will only be to confirm your employment with the company listed above.
    By listing you reference above you are giving SPAAR approval to contact that person; we encourage you to let your reference know that a SPAAR representative will be contacting them.
    Applications submitted without references will not be reviewed.

    Disclosure Information

    By submitting this adoption application, I agree that all information I have provided is comprehensive and true to the best of my knowledge. I understand that any misrepresentation may result in SPAAR refusing to allow me to adopt now and in the future. If, for any reason, the information I have provided changes, I will immediately contact SPAAR and update the contents of this application. I authorize a representative from SPAAR to contact any references I have listed, if applicable. SPAAR agrees to keep all information provided on this application private and confidential.
